FULL CONSENT TO PARTICIPATE IN AN ONLINE SURVEY

Oakland and Berkeley Carsharing Survey

You are being asked to participate in a research study led by researchers at the University of California, Berkeley. You were selected as a possible participant in this study because you are a member of a carsharing service that operates in Oakland and Berkeley, CA.


PURPOSE OF THE STUDY

Researchers at the University of California, Berkeley are conducting this research to gain information about the impact of carsharing on travel behavior and choice of mode.


PROCEDURES

If you volunteer to participate in this study, we would like to ask you to complete the online survey.


POTENTIAL RISKS AND DISCOMFORTS

Some of the questions may make you uncomfortable. You may choose to not answer any questions that make you uncomfortable.


POTENTIAL BENEFITS TO SUBJECTS AND/OR TO SOCIETY

You may not directly benefit from your participation in this research. The results of the study will provide researchers with an understanding of the impacts of carsharing in Oakland and Berkeley.


PAYMENT FOR PARTICIPATION

In appreciation of your time, at the end of the survey, you will have the opportunity to enter your email address into a drawing for a $20 Amazon.com gift card. Your email will only be used to send you the Amazon gift code. Your email will not be shared with a third party and will be deleted from our system after the gift codes have been distributed. The drawing will be held periodically in late 2018. A random drawing for a 1 in 20 chance to win a $20 Amazon.com gift codes will be conducted. You may only complete one survey and enter the drawing for a $20 gift certificate once.


CONFIDENTIALITY

You will not be identified in this study, and no information that is obtained from this survey will be connected with you. Your confidentiality will be kept to the degree permitted by the technology being used. No guarantees can be made regarding the interception of data sent via the Internet by any third parties. Data may exist on backups or server logs beyond the timeframe of this research project.

You may withdraw your consent at any time and discontinue participation without penalty. You are not waiving any legal rights because of your participation in this research study.
