Survey Consent Form

You are being asked to participate in research conducted by researchers at the University of California, Berkeley. You were selected as a possible participant in this study.

PURPOSE OF THE STUDY

Researchers at the University of California, Berkeley Transportation Sustainability Research Center are conducting this research to gain information about your travel behavior and use of transportation services.

PROCEDURES

By completing the online survey, you are volunteering to be part of the study. The survey should take between 5 to 15 minutes to fill out. Unless otherwise stated, you must be 18 years or older to take this survey. By clicking [I agree to take this survey] you confirm that you are 18 years of age or older and consent to participate in this research.

POTENTIAL RISKS AND DISCOMFORTS

You may choose to not answer any questions that make you uncomfortable or stop taking the survey.  Standard risks exist with respect to transmitting information over the internet. The survey collects data using a Hypertext Transfer Protocol Secure (HTTPS) connection, which encrypts transmitted information. 

POTENTIAL BENEFITS TO SUBJECTS AND/OR TO SOCIETY

You may not directly benefit from your participation in this research. The results of the study will provide researchers with an understanding of how people travel and the impacts of transportation systems that may benefit the general public.

CONFIDENTIALITY

You will not be identified in this study. Your confidentiality will be kept to the degree permitted by the technology being used. No guarantees can be made regarding the interception of data sent via the Internet by any third parties. Data may exist on backups or server logs beyond the timeframe of this research project. If an incentive is offered and UC Berkeley is the party responsible for providing that incentive, we will need an email or phone number to conduct the drawing and transmit any incentive. In such cases, you will be required to consent to providing us with this contact information. You can decline consideration for any incentive and participate in the study without providing any contact information.

The compensation for this research, if provided, is described in the introductory text and/or consent page within the online survey.

You may withdraw your consent at any time and discontinue participation without penalty. You are not waiving any legal rights because of your participation in this research study.
