Consent to Participate in Research

FTA MOD Sandbox Evaluation

This survey is about your travel patterns, including your use of Via to Los Angeles Metro stations in the LA region.  Your consent to participate in this research is voluntary.  The purpose of the research is to evaluate how the presence of Via in the LA region has impacted your travel.  You may discontinue taking the survey anytime; you may skip any questions that you are not comfortable answering.   Minimal but standard risks associated with taking a survey on the internet, including data transmission and storage, apply to this survey.  You will not benefit directly from your participation.  You may receive indirect benefits as result of contributing to public knowledge about the performance of this project. 

You are being asked to take this survey because you have used Via in the LA region.  The survey should take about 10 to 15 minutes to fill out. Even if you have not taken Via to Transit in the past, your feedback is still useful. Via will not have access to individual responses, except any anonymous and constructively written comments about its service provided in just the last question of the survey (this question will be noted). The aggregated analysis of this data will be public when complete.  

The study is being led by ICF, a consulting and technology services firm, and the University of California, Berkeley as part of the Independent Evaluation of the Federal Transit Administration’s Mobility on Demand (MOD) Sandbox Program.  The lead researcher on this study is Elliot Martin, PhD, the Principal Investigator is Susan Shaheen, PhD, both of UC Berkeley. Researchers from King County Metro, LA Metro, and Sound Transit are also participating in the analysis of this data.  Data will be classified as confidential and kept as such within these agencies to the extent permissible by law and technology.  De-identified data from this survey may be used for future research by the previously defined project evaluators and agencies without additional informed consent.  Whether or not you choose to participate, to answer any particular question, or continue participating in the project, there will be no penalty to you or loss of benefits to which you are otherwise entitled.
